Consequences of badly fitting dentures

We usually recommend a Denture supported by at least 2 implants as the minimum standard of care.

For the elderly, the choice of treatment should be simpler and efficient which requires low maintenance. In developed countries, a “Removable Implant Supported denture on minimum 2 implants” is the basic standard of care. Though they are retentive, doesn’t aid in the function. The most comfortable and functional solution to provide a new set of teeth would be a Fixed Implant Supported Prosthesis on 4 to 6 implants”. It is nothing but a set of teeth on a fixed frame that takes the support from the implants placed inside the jaw bone.

The better the foundation, the stronger would be the structure. As we increase the number of implants (between 4 to 6), the support gets stronger, the tooth frame feels fit and looks much natural as it has a lesser amount of Acrylic (plastic). It feels comfortable and increase confidence. It also prevents you from looking prematurely old by preventing damage to the jaw bone. A lesser amount of acrylic (Plastic) in the base makes it easier to talk and speak clearly.

Treatment planning is extremely important and so, we first make the diagnostic denture and suggest you go for the 3D CBCT scanning. Once we know your expectations and have all your diagnostic data; we plan the treatment, number of implants and the type of final prosthesis on digital software. During consultation, we usually offer 2 best solutions and discuss them in detail.

Though you don't have to go in much technicalities of this, as a part of our patient education we would like to give you some idea about the factors influencing the treatment costs. The FMR costs depend on factors like -

  • Bone Related: The type of bone Quality, Quantity and Density does affect the treatment planning. When the available bone is insufficient, to place the ideal sized implants, additional surgeries like bone grafting and sinus lifts are done which makes the treatment bit expensive.
  • The Overlying Gum Health: For the long term success, it is the 2nd most important parameter to be taken into consideration. Gum Tissue Augmentation is performed if the tissue health is inadequate and only skillful surgeons like Dr. Desai (Periodontist) can perform the same.
  • Patient's need and expectations have to be conveyed well before planning the treatment. A minimum of 2 implants supported denture is the basic standard of care. Though it doesn’t aid in the function, but it is well retentive. As we increase the number of implants, the support gets stronger, the speech gets clearer, the tooth frame feels fit and the teeth look much natural as it has a lesser amount of Acrylic (plastic). People undergoing "All-on-6" treatment feels much confident.
  • Implant related: The number of implants to be placed depends on your expectations, budget and available bone type. The Implant Company or the brand that you have been recommended makes a huge difference in your estimate. Like Straumann SLActive, BLX or Nobel Active are of elite category whereas systems like Osstem and Neodent comes in mid-range.
  • Prosthesis related: The type of the prosthesis, the material used and the lab in which it was made makes the difference. Implant prosthesis is the final set of tooth frame which is fixed on the implants. Your dentist will suggest the 2 best options which suit your case. Usually, the base framework is made from either Zirconia, PEEK, Cobalt-Chromium or Titanium material which are layered with Acrylic, Ceramic or HIPC Composite. CAD-CAM Milled Metal frameworks are always expensive than the casted one.
  • Surgeon's Skill Charges: The surgical risks involved, expertise and experience of the dental surgeon influence the treatment cost.
  • It also depends on the number of appointments required.
